Employees and covered dependents who are enrolled in a medical plan option administered by Blue Cross and Blue Shield of Illinois are eligible for the Ascension Medical Group CPC Primary Care+ clinic. You can now use the clinic and see other in-network doctor(s). The clinic can be a complement to your health care experience or provide all your primary care needs.
Employees and covered dependents who are enrolled in one of the CPC medical plan options administered by Aetna must use the Ascension Medical Group CPC Primary Care+ clinic for all their primary care needs.

Eligibility
You are eligible if you meet the following criteria:
- You are enrolled in a Boeing-sponsored medical plan option administered by Blue Cross and Blue Shield of Illinois, or enrolled in one of the Aetna CPC medical plan options, and
- You are an active employee or early retiree.
If you are eligible, your covered spouse or domestic partner and covered dependents are also eligible.
